The Case Manager serves as the central hub of client care at The Ark. This role is responsible for developing collaborative care plans with clients and connecting individuals and families with both internal and external resources. Case Managers support clients through active listening, motivational interviewing, and basic crisis intervention, while coordinating care across multiple service areas.

Responsibilities / Duties

Case Management and Care Planning (50%)

Serve as the “hub” of client care, collaborating with other teams and departments.
Create and update collaborative care plans with clients that align with identified needs and goals.
Provide active listening, motivational interviewing, and basic crisis intervention to support client stability and self-reliance.
Maintain accurate and timely records
Provide referrals to external community resources and services as appropriate.
Monitor progress and follow up with clients to ensure connection and engagement with referred services
Provide assistance in applying for public benefits such as SNAP, Medicaid, LIHEAP, and SSI
Coordinate communication across internal teams to ensure cohesive, client-centered care.

Cash Assistance Coordination (45%)

Process requests for non-housing related cash assistance and coordinate with other members of the care team to determine appropriate allocation
Follow the check request procedure to disperse cash payments on the clients’ behalf
Other responsibilities (5%)

Participate in team meetings, trainings, and professional development.
Assist with program development and improvement efforts as needed.
Qualifications

Required:
Bachelor’s degree in Social Work (BSW) or related human services field. MSW Preferred.
Preferred: Continuing education or training in motivational interviewing, trauma-informed care, or crisis intervention.
Minimum 1–2 years of experience in case management, social services, or related human services role.
Experience working with individuals and families navigating multiple stressors (economic, medical, housing, or psychosocial).
Familiarity with community resources and referral processes.
Knowledge of basic symptoms of mental illness and ability to screen/assess for referral purposes (without diagnosis or treatment).
Proficiency in Microsoft Word and Microsoft Teams.
Strong interpersonal skills, with ability to build trust and rapport in diverse communities.
Skilled in active listening, motivational interviewing, and crisis de-escalation.
Ability to collaborate effectively across multidisciplinary teams.
Excellent organizational and documentation skills
Deep understanding of Jewish values and cultural humility
